# Sign-up Forms on Members Portal 5

### What are Sign-up Forms?
{{variable.Customers_CAP}} need to provide information when they create an account on the {{variable.MembersPortal}}. You can customize your sign-up forms to control what information you collect from {{variable.customers}} when they sign up and when they book a tour. 

Sign-up forms also control whether [guest checkout](/v3/docs/guest-checkout-on-members-portal-5){target=`_blank`} is enabled in your space.

## How do Sign-up Forms work?
There are two types of sign-up forms: the **default form** and **extended forms.**

The **default form** is used in all sign-up flows. It also controls whether [guest checkout](/v3/docs/guest-checkout-on-members-portal-5){target=`_blank`} is enabled in your space. 

The default form includes **three key fields**:
* [customer type](/v3/docs/customers#customer-types){target=`_blank`}
* name
* email

You can [customize the default form](/v3/docs/customizing-the-default-sign-up-form){target=`_blank`} by disabling the {{variable.customer}} type field and adding any additional fields you'd like {{variable.customers}} to fill out when they sign up. 

*For example, you may want all {{variable.customers}} to provide their billing details when creating an account.*

The **extended** forms let you add fields for sign-up flows that include specific purchases. You can also customize sign-up forms for {{variable.customers}} who are joining {{variable.teams}} and your tour request form. 

*For example, you may want {{variable.customers}} signing up to a {{variable.plan}} to include additional contact information.*

* Purchasing plans
* Making bookings
* Purchasing products
* Purchasing event tickets
* Enrolling in courses
* Joining teams
* Booking tours 

## FAQ
### What happens when a customer purchases items linked to different extended forms in a single transaction?
When {{variable.customers}} make a purchase that includes items linked to different forms, the system automatically shows the additional fields you've added for all purchase flows.

*Let's say purchasing an event ticket requires a **date of birth** and a **phone number**, and purchasing a product requires a **company name** and a **phone number**.*

*A {{variable.customer}} buying an event ticket and a day pass bundle in the same transaction would add the following details: **date of birth**, **phone number**, and **company name** in addition to the default sign-up form fields.*
